ART THERAPIST

Utilizes art media and the creative process to help patients cope with end-of-life issues and to enhance quality of life. 
Requirements:  Master’s degree in Art Therapy from an AATA accredited program or Master’s degree with emphasis in Art Therapy;  registered or currently obtaining hours for registration/board certification; one to two years’ experience in the field or Art Therapy internship in hospice.

BEREAVEMENT COORDINATOR
Provides bereavement follow up to the Hospice of the Western Reserve families after the death of a loved one.
Requirements:  Master’s degree in social work or counseling preferred or a bachelor’s degree in social work/counseling with one to two years’ experience in group work and counseling; clinical experience or skills relevant to grief and loss; ability to problem solve, make decisions and plan programs.

CHILDREN’S BEREAVEMENT COORDINATOR
Provides supportive grief counseling to children who are at risk for complicated grief or are in high-risk family situations.
Requirements:  Master’s degree in social work or counseling; at least one year experience working with seriously or chronically ill children and bereavement issues; prior hospice experience preferred; other related degrees may be considered based on type of experience.

COOK
Under the supervision and direction of the Dietary Services Supervisor, prepares and cooks meals for patients, staff and guests.
Requirements:  Any combination of education or experience equivalent to graduation from high school; dietary/cooking experience in a health care food service environment; a minimum of one year culinary training preferred.

COORDINATOR OF VOLUNTEERS
Under the supervision of the assigned supervisor, coordinates, supervises and offers support to the volunteers in the various capacities and serves as a liaison between volunteers and other team members.  Responsibilities include assisting with recruitment, training and retention of volunteers.
Requirements:  High school diploma; prior experience as a volunteer or in supervision of volunteers preferred.

DIETARY SERVER
Under the direction of the Dietary Services Supervisor, prepare and serve meals, set up and transport patient trays and food carts, wash dishes and clean/sanitize kitchen area.
Requirements:  Must be able to read, write and carry out instructions.

DIETARY SERVICES SUPERVISOR
Supervise dietary and housekeeping services and staff.
Requirements:  High school diploma or equivalent; associate degree in related field or comparable work background in supervision with a minimum of two years’ experience.

EXTENDED CARE TEAM SUPERVISOR
Responsible for the direct supervision of the extended care staff.  Act as a resource to team members to provide clinical guidance according to policy and procedure and address questions and concerns with follow up as needed.
Requirements:  Licensed RN in state of Ohio; BS or equivalent experience preferred; leadership experience or demonstrated ability to lead.

HELP DESK COORDINATOR
Provide support to end users on issues related to performance, maintenance and use of the agency’s personal computers, networks, applications and Access Databases.
Requirements:  Two years post high school education and two years’ work experience in help desk, database support and data conversion or related work; Citrix experience a plus.

HOME CARE NURSE
Provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in a variety of settings.  Coordinate the implementation of the Plan of Care for each patient.
Requirements:  Licensed RN in state ofOhio; experience preferred in pain and symptom management, oncology, public health, mental health; must have valid driver’s license, car availability, and insurance coverage as required inOhio.

HOSPICE NURSING ASSISTANT
Provide personal care under the supervision of a nurse according to the patent’s plan of care.  Collaborate with transdisciplinary team in a variety of settings.
Requirements:  Must be able to read and write English proficiently; must have 12 months experience as a nursing assistant; must be state tested and in good standing; have completed a certified Home Health Aide Training Program including 16 hours of classroom experience and 75 hours of clinical experience.

HOSPITALITY AIDE
Perform housekeeping duties in assigned areas.
Requirements:  Must be able to read and write English and carry out instructions.

INTAKE ASSISTANT
Receives referrals and inquiries about referrals.  Collects and inputs data and confirms appropriateness of referral.  Consults with assigned Patient Benefits Coordinator and/or Managed Care Coordinator regarding insurance coverage.
Requirements:  Medical Assistant preferred; high school graduate or equivalent; one to two years’ experience in medical office or health care agency preferred.

LIAISON NURSE
Provides an overview of hospice services within the home, hospital or alternative home setting and facilitate the admissions process when appropriate.  Coordinate implementation of an initial plan of care based on immediate needs of the patient/family unit.
Requirements:  Licensed RN in state of Ohio; BSN preferred; experience preferred in pain and symptom management; oncology; public health or medical/surgical background.

MEDICAL RECORDS COORDINATOR
Under general direction, performs medical record/chart maintenance.  Maintains accurate and complete records, and assures compliance with agency standards and Medicare Conditions of Participation and HIPAA regulations.
Requirements:  High school diploma; knowledge of medical terminology; knowledge of the principles and practices of health information records management; ability to maintain accurate records and files.

MUSIC THERAPIST
Provide music therapy interventions and services to patient.
Requirements:  Board certified Music therapist; at least one year experience in the field of music therapy or a music therapy internship in hospice care or extensive experience working with the terminally ill.

ON CALL SUPERVISOR
Responsible for the supervision of the On Call staff under the direction of the Director of Home Care.
Requirements:  Licensed RN in state of Ohio; BS or equivalent experience preferred; leadership experience  or demonstrated ability to delegate and manage people and projects/programs.

PAYROLL SERVICES COORDINATOR
Perform all aspects of the payroll functions including reporting, maintenance and processing.  Assist in preparation of all monthly payroll, payroll accrual and related expenditure journal entries along with the appropriate support schedules.
Requirements:  Associate’s degree in Accounting;  two to four years’ experience with processing payroll along with general accounting experience; two to four years’ experience with a computerized payroll and accounting system; background in ADP and MAS90 preferred.

PHARMACIST
Provide quality, cost effective pharmaceutical care to hospice patients.  Provide drug information, education and other needed services to enhance drug therapy outcomes.
Requirements:  Current Ohio pharmacist’s license; PharmD preferred; minimum four to five years’ experience in a clinical setting.

RECEPTIONIST
Greets and directs visitors in a friendly, professional and helpful manner.  Supplies information to the general public, staff and patients/families.  Conveys in a cordial and professional manner, the philosophy of the agency.  Supports the office and team members by keeping the integrity of the agency, providing assistance to team members and answering office phones in a timely manner per agency standards.
Requirements:  High school diploma; two years’ experience performing receptionist functions, preferably in a healthcare environment; proficient in Microsoft Word, Excel and Outlook.

RECRUITMENT AND RETENTION COORDINATOR
Under the direction of the Human Resources Director, is responsible for leading the recruitment efforts of the organization.  This position is responsible for job fairs, advertising, screening applicants, interviewing, candidate searches via internet recruiting tools, and management of the applicant tracking system.  This position also assists with the retention and development of our agency staff.
Requirements:  Associate or Bachelor’s degree or equivalent experience and an achievement-oriented recruiting professional.  This position requires excellent verbal and written communication skills and knowledge of employment and diversity laws.

REGISTERED NURSE
Provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in our in-patient facility.  Coordinate the implementation of the Plan of Care for each patient.
Requirements:  Licensed RN in State of Ohio; experience preferred in pain and symptom management, oncology, public health, mental health.

SHIFT SUPERVISOR
Responsible for the overall functioning of the facility and for the direct supervision of staff on a specific shift.
Requirements:  Licensed RN in state of Ohio; BS or equivalent experience; leadership experience or demonstrated ability to lead.

SOCIAL WORKER
Provide medical social work and counseling services to terminally ill persons/families in collaboration with the transdisciplinary team in a variety of settings.
Requirements:  Masters prepared social worker; licensed in the state of Ohio; 2 years experience in counseling or related work with individuals, families and groups; experience preferred in hospice, medical facilities or nursing homes.

TRIAGE NURSE
Respond to the needs of patients and/or caregivers during the hours when primary team members are unavailable.  This may be done via telephone communication and/or a home visit.
Requirements:  Licensed RN in state of Ohio; experience preferred in pain and symptom management, oncology or mental health; able to turn, transfer and/or lift up in bed a patient of any size independently.

UTILITY/SECURITY/MAINTENANCE
Provide building operations, security and maintenance services.
Requirements:  Must be able to read, write and carry out instructions.

Applications are received and sent to the respective supervisors for review. The supervisors contact the applicants and ask them to come in for an initial interview. Promising candidates are then brought in for a second interview. The final candidate will receive a job offer from the hiring supervisor.
