Hospice of the Western Reserve is a community-based non-profit organization providing comprehensive hospice services for those individuals with a serious illness. Several clinical studies have demonstrated the benefit hospice and palliative care can have for patients diagnosed with dementia, cardiopulmonary disease, kidney disease, cancer and other terminal diagnoses. We are able to provide services to patients with any diagnosis, at any age (including our pediatric and perinatal programs) regardless of their ability to pay.
Through our Western Reserve Navigator program, Hospice of the Western Reserve partners with patients to manage their pain and other symptoms for those whose diagnosis is chronic or those not yet ready for hospice care.
We are also able to offer support and direction for patients and families interested in advance care planning and ethical wills.
Hospice of the Western Reserve team members collaborate with patients, families, caregivers, referral sources and other healthcare professionals to provide holistic support for patients throughout Northeast Ohio. We are always available to answer any questions you have.
