Levels of Care

To meet you or your loved one’s varying needs, we have different levels of care. The full array of hospice services are offered at each level, regardless of where services are provided—private home, nursing facility, hospital, in-patient unit—wherever he or she calls home. We welcome participation in the care plan to find the level of care that is best suited for you or your loved one.

Western Reserve Navigator Palliative Care Services

If you or your loved one is in the early stages of a serious illness or is living with chronic pain, our palliative care program may be an option. Palliative care focuses on easing symptoms and reducing anxiety, so he or she can live their life to the fullest. Learn more…

Home Care

At this level, a member of the hospice team makes regular visits to the home to assess you or your loved one and provide additional care or other services. If there is a brief, acute episode that requires additional care to manage pain or acute medical symptoms, nursing care can be provided on a 24-hour continuous basis to maintain the patient at home. Hospice Care Settings

Patients may be admitted to David Simpson Hospice House or Ames Family Hospice House for intense pain and symptom management, residential care or to provide respite for caregivers. In some cases, you or your loved one may stay in a hospice care unit at the hospital. Learn more…

Pain and Symptom Management

Often referred to as General In-Patient or “GIP,” this level of care is used when there is an episode of unbearable pain or acute symptoms such as shortness of breath, swelling of hands or feet or fever.
