What to Expect in the First Few Days After Contacting Hospice Care

At Hospice of the Western Reserve, we want families to feel confident and informed from the very first conversation. Here is what you can expect in the first few days after contacting us.


Initial Conversation and Information Gathering

During your first call, we will listen carefully to your needs and answer questions. We will talk about your loved one’s condition, current care and goals. This conversation helps us understand how we can best serve you and prepares us for the next steps.


Evaluation and Admission Process

After the initial call, we will schedule a visit to your home or care setting as soon as possible. The team member will complete a thorough assessment of your loved one’s symptoms, medications and overall condition. During this time, they will also explain hospice services and review what to expect. 


Creating a Personalized Care Plan

Once admitted, the hospice team creates a care plan tailored to your loved one’s needs. This plan includes medical, emotional and spiritual support for both the patient and family. A nurse will manage medications and coordinate with your doctor. Social workers, aides, and volunteers will provide added support to make daily life easier.


Ongoing Communication

From the first day, we keep you informed. Our team checks in regularly, and you can call us at any time. We want you to feel supported every step of the way.
